iCommunity Connection Services, Inc. provides coordinated care for Veterans and their families by serving as a single point of contact between them and local government agencies and other nonprofit organizations.
In addition to aiding Veterans and their families with navigating multiple agencies and organizations iCCS assists by:
- Identifying the individual Veteran’s needs;
- Identifying local agency or organization’s that provide services;
- Acting as sole Case Manager in interfacing with agencies and organizations;
- Coordinate all referrals with agencies and organizations;
- Assist with making appointments and coordinating transportation when needed;
- Assist individuals with the goal of moving towards stability and independence; and
- Ensuring Veterans needs are understood by local agencies and organizations.
